Dopo che una guerra nucleare ha trasformato la terra in una distesa arida ed irradiata di rovine, alcuni superstiti trovarono la salvezza in profondi bunker sotterranei chiamati Vault, resi funzionali dalla tecnologia della società Vault-Tec. Il protagonista vive nel Vault numero 101. Il gioco vero e proprio (a parte tre brevi prologhi a 1 anno, 10 e 16) ha inizio quando, a 19 anni, il protagonista lascia il Vault 101 alla ricerca del padre, James, uno degli scienziati più rispettati della comunità che è scappato nel mondo esterno. Uscito dal Vault il protagonista scopre che le radiazioni nel mondo esterno sono drasticamente diminuite, permettendo così la sopravvivenza di alcune comunità di esseri umani.

Una di queste era Megaton, insediamento umano costruito nel cratere lasciato da un bombardiere precipitato. Alcuni edifici sono stati costruiti con i pezzi del velivolo uniti con altri pezzi recuperati da un vicino aeroporto. La testata nucleare che trasportava è rimasta inesplosa al centro del cratere, diventata col tempo parte della città e, da alcuni, addirittura oggetto di culto religioso. In questa città il protagonista scopre che il padre si è diretto alla sede di Galaxy News Radio, una delle uniche due emittenti radiofoniche ancora esistenti. Il suo capo è un disk-jokey chiamato TreCani, che si pone come voce degli oppressi della zona contaminata.
